We look forward to seeing you soon. For your convenience, you have two options for scheduling an appointment:
- Visit the Center for Healthy Living patient portal to request an appointment. If you are a new user, you will create an account using an email and password of your choice. Once your account has been established, log in to request an appointment.
- Call 765-494-0111. When scheduling your appointment be sure to let them know you want your appointment at the Fort Wayne location.

First-time visitors to the Center for Healthy Living should complete the following forms and bring them to their first visit:
- New Patient Health History Form
- Patient Information Sheet
- HIPAA Information and Privacy Consent Form
- One to One’s Notice of Privacy Practices & Acknowledgement
Patients requesting the release of their medical records should complete the following form:

Purdue medical plan participants should bring their Anthem Member ID Card. All other eligible patients should bring their Mastodon Card (if applicable) and their health insurance member ID card.
Patients should also bring all applicable forms (see the previous question).
The center accepts all major credit cards. The center does not accept checks or cash.
- Physical examination
- Biometric screening (weight, height, blood pressure, LDL, cholesterol level, and A1c)
- Depression screening
- Skin cancer check
- Flu shot
- Prostate exam
- Colorectal screening
- Pap smear
- Shingles vaccine
- If you completed your physical and biometrics at the clinic and have a registered Healthy Boiler account, the Center for Healthy Living will upload your form into the Healthy Boiler portal as a concierge service, if you wish. Or you can upload the form yourself into the Healthy Boiler Portal.
- If you completed your biometric screening at the clinic and your physical form has already been uploaded into the Healthy Boiler portal and approved, the clinic will ensure that your biometric values are entered into the portal on your behalf. No manual submission on your part is necessary.
- For all other Healthy Boiler incentives that you completed, go to your Healthy Boiler account, locate the screening that you completed, click Submit, and check the Yes box to indicate you completed that item.
